Abstract

Background and Purpose: Adhesive Capsulitis, also known as frozen shoulder, affects about 2- 5% of the general population and yet the exact cause is still unknown. It occurs when the capsule of the glenohumeral joint thickens and causes the joint to become stiff resulting in decreased range of motion, strength, and pain which all can last for multiple years. The purpose of this case study is to describe the clinical reasoning and decision making in the physical therapy management of a patient with adhesive capsulitis by evaluating and discussing the examination and evaluation, interventions, and overall plan of care in an outpatient setting. Case Description: The patient was a 43-year-old-female with symptoms that included: pain with overhead activities and getting dressed, decreased range of motion, and decreased strength. She was an active individual who owned her own yoga company and was very motivated to improve her function.

Interventions: A combination of manual therapy and therapeutic exercises in conjunction with a corticosteroid injection were used in this case. These interventions were utilized to decrease pain, improve range of motion, strength, and improve overall function with daily activities.

Outcomes: Over the course of therapy, the patient was able to decrease pain, increase shoulder range of motion, strength, and become more functional in daily activities.

Discussion: The patient responded well to treatment and met her goals. Her progress was in line with current literature. However, more research needs to be done to determine the most beneficial treatment for effectively rehabilitating adhesive capsulitis.
